Patients use connected devices at home – like heart monitors and blood pressure cuffs – that automatically upload their health data through the cellular network to their doctor. Many patients already take steps daily to help combat their chronic health issues, but Wellealthy can make sure no one misses a measurement…literally.
Here’s How it Works

- Sign up quickly and easily with MD Care.
- Enroll your patient(s) by providing EHR access or simply providing a file. Our system integrates directly with Medicare and and provides information on what devices and services a patient is eligible for. We take care of outreach and patient education.
- MD Care will ship your patient their device to their home.
- Our highly trained and board certified doctors review your patient’s device readings in the MD Care portal. Staff are automatically notified if a patient’s readings enter a specified caution or critical range, and reports can be customized to best suit your workflows.
- Get paid. MD Care will revenue share with your practice for each qualified Medicare patient. Depending on the qualifying factors, you can expect $15 – $20 per patient, per month.
